New Patient Information
Welcome to our office! This is the first step in restoring your overall health. Your first appointment at our office promises to be a pleasant experience. My staff and I are well trained and will make you feel very comfortable.
A complete examination of your teeth, gums and bone will be done. Your periodontal pockets will be checked. An oral cancer screening is also performed and blood pressure is taken. Dr. Miller will evaluate your x-rays from your dentist, or take a complete set of films, if necessary. Please feel free to bring a list of questions to your first appointment.
Please assist us at the time of your initial visit by providing your referral slip and x-rays, if you have them. A referral is not needed to come to our office. Please provide us a list of your medications that you are taking. In addition, if you have dental insurance, please bring any forms or insurance cards with you to the appointment.
